1. Definitely not there. Used three different chars and checked during many different missions and patrol.
2. I don't remember if there was a prompt. But I do remember picking it up I think.
3. I'm pretty sure I picked it up during the Alpha or Beta as it was in a easy to find area. I played both pre-releases. But I don't remember for sure. If not, it definitely was the very first week of the games release.
