You're playing through Crota's End on the Crota CP. There's a Defender Titan that distracts boomers with his bubble. Everything is fine until Crota enrages. The swordbearer somehow dies (Lol) The Titan runs up to Crota, tells everyone to shoot, grabs the sword, and kills him. I don't even know if this is possible. Lol.
