While turning in bounties i leveled up a weapon to max rank, without leaving I upgraded it in my menu and now its maxed out but it still has the green progress bar under it instead of the yellow border.

Has it been ascended? Thats wat happens for all weapons and armor. U may have all the upgrades on it but until u ascend it the green bar is there.

It's fixed, I wasn't aware that the black hammer was bugged.
