Looks like you only need to find one more gold chest and to do Prison of Elders on Hard (lv35)

For year one players you have to complete a series of tasks to get an emblem. From what they said those with the emblem will get a set of armor and some other goodies but none has been confirmed what we are actually getting. To see what you need done, look on the bungie site under your profile, then click triumph. Or you can click your name when looking at your characters on the app, and right below you will see moments of triumph. It will show you your progress.
