More than likely Guardians with max grimoire score acquired the ghost you're referring to and the double grimoire card before they became glitched.

They weren't always glitched. If I remember correctly, they became glitched when the House of Wolves came out. Doubles was glitched when it first came out, then was fixed, and now is glitched again. Fun! : )
