Personally?
CQB is the ugliest. It looks like all the left over parts of the other helmets all mashed into one.
Closely followed by...
*put up flame shield*
Mjolnir MK V base. Too basic, plus the visor looks doofy.
My favorite is operator though, all the equipment looks a bit like Sam Fisher.
