Hello,
Can you ensure you're checking this exact location for the ghost
Ghost fragment: fallen

You have all other ghosts (including the last word 3)
As for the race grimoire, you need to make an awoken character to unlock that grimoire.

Thanks for your reply, Yes i have checked the location for the fallen dead ghost. I have found all the current dead ghost in the game.

While destiny is open, try logging out of your profile (next to where you change character). This sometimes unlocks grimoire. If you can check that location again.
