I was streaming on Twitch the other day, went to the bathroom with my headset on and forgot to mute my mic.
First comment in the feed I see when I get back is:

Had a similar situation where my controller was muted, but I guess I was too far from my Xbox in the bathroom so it turned off...then back on...and mute was no longer on...
