I could be wrong, but I believe you cannot get it from a quest.

I believe your right. All I've seen so far are just engrams I believe.

I got both from three of coins farming. It drops just like any other exotic I assume. Engrams, nightfalls, raids, xur (maybe), 3 of coins, etc.
