Hey was playing iron banner and since taken king came out ive been seeing things like me shooting a guy, mostly happens with shot guns and then dying instead or getting no heavy ammo when i pick it up, or hitting someone with grenades or bullets and them regenerating healt instead of loosing it.

I've had the health meter regeneration effect happen to me all the time. I think I've killed an enemy only for them to have their health meter to regenerate to half and have to kill them again.
