I really don't know what to tell you. Maybe there's a boss you haven't beat yet that you need for it? Or just beat it a few times and see if that's it. The card is unlocked for me and just tells me how many completions and vip kills I have. Doesn't have a checklist or anything.

Have you beat all the lower level bosses too? Because it sounds like you should have it. Also, is this all on the same character? The fragments are not shared like the ghosts. They're like the Golden Chests, each needs to find them.
