I believe Dreg/Vandal/Captain is a sort of experience / skill thing. Fallen only really grow to Captain size pretty late in life, but most of the Vandals and Captains are pretty young. When deemed worthy, eliksni get the docking caps taken off of their lower arms, and are given doses of a substance known as ether. This substance makes them grow faster and bigger, giving them the appearance and strength we see.
