some ghost still aren't showing up in world so I can't get the grimoire cards. example: "mystery: vault of glass 3"

If you wipe during the Paradox Daily Mission the remaining Ghosts dissapear and you have to restart. See the guide for more info on that - https://www.reddit.com/r/DestinyTheGame/comments/3t9v5o/daily_heroic_is_paradox_again_grab_those_mystery/
