The ghost fragment that you find in a cave on the way to the Forgotten Shore on Earth has never unlocked, I collected the ghost months ago but never got the score, it didn't count as me collecting it and I can't pick it up again

Try heading back and checking this location again. Some users had troubles picking it up for a while.
