Right now, I'm looking at a bug within Destiny's nightfall. The door in the Asylum which leads into the Alak-Hul strike will not open. The door into the arena where the Guardians face Ta'aun, Hand of Oryx and Baax, Hand of Oryx is open, no Taken will spawn and the two Ascendant Shriekers from the previous section are still in their place, Immune, and cannot be provoked. The Ascendant Shriekers, from my honest opinion, are the source of the entirety of the bug in the Alak-Hul Nightfall strike.
