The "insight" grimoire won't unlock. I have ALL fragments on my warlock and most all on my other two characters. Grimoire still isn't unlocking it. Any others with this issue?

Can you double check the in game counter on the character you believe has collected 50 calcified fragments? To see the counter select the Dreadnaught destination from orbit. There is a counter in the lower right of the map.
