You have to have a treasure key to get it. It comes from the chest.

One of the guys i just ran it with got it out of the small chest on the left :(

Impossible. It only drops from the big chest. Look it up if you dont belive me.

Ok thanks for the info. He did open the big chest but claimed it came from the small one.
