The only etiquette that should be adhered to is to get there on time. Don't get me wrong, if you're a few feet away, I will wait if the coast is clear. But if you're not around when it comes up then that's your fault. I want to get in position asap so I can pick off the other team and maybe make them pay for being stupid enough to wait. On some maps getting to a certain spots with heavy is a huge advantage and I'm not gonna give that up for randoms. If you value heavy that much and you didn't get there in time, chances are (not always but probably) you weren't worth waiting for. Not getting heavy doesn't bother me that much because I can actually get kills without it.
