We were doing CoE the week they had Melee and Exposure. It was the Seditious Mind.
Me: Whos down to smack him?? He's low enough.
Them: *silence*
Me: Oooookay...*runs up, smack boss, boss explodes, we all die*
Me: The -blam!- just happened??
Teammate 1: I was going for the melee kill!
Teammate 2: Me too!
Me: *sighs and rubs forehead*
Teammate 1: Yo we gotta kill this dude AGAIN???
Me: Yup. We all died at the same time.
Soon as we ran back out, I sprint towards the platform and die.
Teammate 1: Kee!! What the....ah you skank. You got an exotic??
Teammate 2: I got one too! Hol' up!

Me and a buddy were doing the servitor one- we were on pace for a nice score (45K+)- my buddy died right as the boss died. I ran up to revive him, and tinking mines killed me. We had to do the whole fricking thing over again.....
