Incompetence is the headlining issue: they made an average game(stick your Dink in, now defend), and then eroded it over the next few years.
Great idea that fell into the lap of an incompetent developer.

100% agreed. People try to defend Bungie, saying "They've never done a game like this, of course there will be some missteps"... Bullshit. Gearbox Software has demonstrated their own struggles with competency (Duke Nukem, Colonial Marines). Yet when they opted to create an open world looter shooter with rpg elements (sound familiar?), which was something that they'd never done before, they didn't make a fraction of the mistakes that Bungie has made. They succeeded for two simple reasons: 1. They made sure that fun [i]always[/i] came first, and; 2. They understood that competitive PvP has no place in an rpg-inspired looter shooter. If you want your game to be first-and-foremost [b]FUN[/b], those two ideas are incompatible. Bungie's desire to be 'all things to all players' has left their game in the mundane state of being a jack of all trades and master of none. Whatever it is that a person enjoys about Destiny, there's probably another game out there that does it better.
