I recently picked up a Limited Edition PlayStation 4 Pro 1TB Console with Destiny 2 and I never got my Coldheart Exotic or Kill-Tracker Ghost after I completed the campaign and reached level 20. Included was the PS4 Pro console + Accessories and Destiny 2 with Digital Content Pack. I entered the code for the Digital Content Pack but was only given access to the "Premium Digital Content Pack" and "Expansion Pass". Yes, I have entered all the codes given in the Physical copy case and checked the box entirely. And yes, I have checked the Gunsmith and Shaxx, restarted the game several times and even reinstalled the game on my PS4.
