In Gambit, there's a very, very, very small chance for an Ascendent Primeval Servitor to spawn. It looks just like the boss from the end of the Forsaken campaign. If you kill this Primeval, you get an exotic quest that rewards the Malfeasance.

For the malfeasance quest to drop you have to kill a special servitor boss that randomly spawns. But since the other team left the couldn’t kill it and so they couldn’t get the quest.
