Wait, so you need to kill the nightmares fast, but the boss slow? That doesnt seem to make sense. Time would run out surely?

When the boss spawns after altar V, you want to only deal enough damage to trigger the first phase (he goes immune and nightmares run for the centre), and you have to do that five times without any sacrifices before you can kill the boss. The idea is to make it really hard to complete the triumph with randomness because they’ll just try to nuke the boss after each phase — and that usually means he’ll die after 2-3 phases. I’ve yet to get it because a match made 3 stack is too small but having five randoms kills it too quickly.

Every nightmare you kill grants more time. So if there are enough people you can down all the nightmares quickly and get enough time I think.
