I did a Flawless run on Pit of Heresy and it didn't reward me the Triumph
I just did a Flawless run on Pit of Heresy from the beginning to the end without dying but it didn't reward me the Triumph for it.

None of your runs qualify in that regard. https://dungeon.report/pgcr/10674865517 https://dungeon.report/pgcr/10667138174 In both of these, you had teammates dying.
