Cannot complete Access Override final puzzle

After inputting the first four numbers to generate the first half of the puzzle solution, all panels fail to reset properly and are impossible to interact with. I have tried swapping to a kinetic scout rifle to ensure that no extra inputs are made, and I have tried resetting the puzzle by wiping, returning to orbit, and completely resetting and rerunning the whole dungeon. After several attempts, the issue occurs 100% of the time - link attached is one instance.
