I've been soft locked in the Nether a handful of times but now that Steam supports background recording I was able to get a clip. I think it happens when you kill the mini DarkBlade before its shield goes up. The ADs don't despawn and the encounter doesn't end or give you progress. I have the full run if needed as well.
