Hey there, 'Fractured by the Witness' is a debuff the fireteam has to manage during the activity. Once it reaches at least x7, then another player can take it from you and it starts to build up on them instead. Otherwise it kills you and jumps to another player. Edit: I saw you were playing it solo, the 'Co-operative Focus' missions are for two or more players, as not only do they feature the Fractured debuff, but also other things that require at least two players - simultaneously shooting two different darkness nodes, 'Berzerker' enemies whose two opposite weak spots need to be hit simultaneously too, etc.
