Impossible to get A rank in Fireteam Ops

I've ran Fireteam Ops over a dozen times, and it's nearly impossible to get B rank, let alone A rank. I've tried matchmade and customized, setting the power delta to over -40. Projected score says B+, but we barely cross C+.
