Dying when changing areas in new activity
Getting soft locked because when we load into the next area in the new activity our team gets killed

I was even doing Presage in the Portal when all of a sudden we got brought back to a slightly earlier point of the mission that we just were at a couple of minutes ago, and it looped killing us repeatedly.
