As we know this game is going to have MMO elements and will be always online so if this game will have difficulties (easy, normal, hard etc) how do you think they will work?

I believe that it will be implemented with the enemy's levels. I think we've seen enemies have level counts, and their difficulty will scale with their levels. Dungeons and raids, like many other shared world games, will be much more difficult possibly implementing an elite-classification of enemies. Now you've got my brain working. Damn you.
