A) The CE ending is non canon. First Strike explains how he survives. However, Bungie had stated they never originally planned to make a Halo 2 at all.
B) Gravemind was on High Charity, not Delta. However, it doesn't explain how he got from HC to the Citadel so quickly. Blame Bungie storytelling.
