I have 'quick scoped' a number of times without issues. I can't see the system you are on and not heard of this problem from other players before. I can only think that, unless you have a controller issue, you will need to make a deliberate attempt to zoom then fire in close order instead of both at once. You can practice on a patrol run.

I'm on PS4 Destiny Edition so it is a brand new controller. It only happens under these circumstances, no other issues.

It could be that in the Crucible you are playing at a more aggressive rate than in any other occasion and should experiment with different input styles. You can also look in the PS forums for insight and advice.
