Are you dead, waiting for re-spawn when you use a Ammo Synth? If so, it's a bug. The Ammo Synth pack will be used, disappear from your inventory, your weapon will not be reloaded and to add injury to insult, you are now stuck with a 5 minute cool down timer before you can use an ammo synth again.
