I don't know for sure but the way I read it the perk goes on cool down once grenade hits a target. So that means if you're using a swarm grenade once one part of the grenade hits, if it doesn't kill the target it won't work for the remaining grenade fragments. If that is how it works then the only grenade that the perk will work for all the time is the tripmine and maybe arc bolt.
